Medication Permission Form

We must have written permission from a parent/carer to administer medication to the children in our care. This permission must be in place before we can give any medications. If a child comes in with medicine, ensure that this form is completed at drop off or sent to the parent to complete before we administer the first dose.

Note: some children in our care will have separate permissions for us to administer Calpol, Nurofen, teething aids or piriton if needed.
